Enrollment is simple.

STEP 2
Patient consent is a required portion of the form. If your patient is not present, he or she can provide consent on their own either:

Submit the enrollment form to YourBlueprint.
(Please do not send the enrollment form to the pharmacy)

STEP 4
Confirm your patient’s enrollment.
Once the completed enrollment form is submitted, your patient’s Case Manager will contact you by phone to confirm your patient’s enrollment and discuss next steps based on the services requested.
