Terms and Conditions of YourBlueprint Alerts Text Messaging

  1.  Enrollment / opt-in to the YourBlueprint Alerts program can be done via online enrollment form. Only patients on a Blueprint Medicines therapy and who have enrolled in the YourBlueprint Patient Support Program are eligible to participate in the YourBlueprint Alerts program.
  2. By opting into YourBlueprint Alerts text messaging, you authorize Blueprint Medicines to contact you by text in connection with the Program services and also with information about Blueprint Medicines’ products, promotions, services, or research studies, and to ask your opinion about such information and topics, including market research and disease-related surveys.
  3. Upon opting in to YourBlueprint Alerts text messaging, YourBlueprint will send you a text message to confirm your signup. You will need to respond to this message in order to be fully enrolled and able to receive text messages through this program.
  4. To receive help with YourBlueprint Alerts text messaging, text “HELP” or call 1-888-258-7768 for further assistance.
  5. You can stop receiving text messages from YourBlueprint Alerts at any time by texting “STOP” to 27973. After sending the message “STOP” to us, we will send you a reply message to confirm that you have been unsubscribed. After this, you will no longer receive messages from YourBlueprint Alerts. If you want to rejoin YourBlueprint Alerts text messaging, please call 1-888-258-7768 to opt back in.
  6. If at any time you have questions about YourBlueprint Alerts text messaging, just text “HELP” to 27973. After you send the message “HELP” to us, we will respond with instructions on how to use our service as well as how to unsubscribe. You can also email info@yourblueprint.com or call YourBlueprint at 1-888-258-7768, with any questions you might have about text messaging and the 27973 “short code” used by YourBlueprint Alerts text messaging.
  7. Participating carriers: AT&T, Sprint/Boost/Virgin, T-Mobile, Verizon Wireless, MetroPCS.
  8. T-Mobile is not liable for delayed or undelivered messages.
  9. Message frequency will vary by user. Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
  10. Blueprint Medicines and its business partners may use and share with each other and with your healthcare providers, pharmacies, and health insurance plans, information about you in connection with providing services to you under the YourBlueprint Patient Support Program, administering the Program, or as otherwise required for Blueprint Medicines to meet its legal obligations. Your mobile number will not be shared outside of the YourBlueprint Patient Support Program.

The value of this program is exclusively for the benefit of enrolled patients and is intended to be credited toward patient out-of-pocket obligations, including applicable co-payments, co-insurance, and deductibles. Patient may not seek reimbursement for the value received from this program from other parties, including any health insurance program or plan, flexible spending account, or health care savings account. Patient is responsible for complying with any applicable limitations and requirements of his/her health plan related to the use of the program. Program is not valid where prohibited by law. Valid only in the United States and U.S. Territories. This program is not health insurance. This program may not be combined with any third-party rebate, coupon, or offer. Offer is not available to patients whose prescription claims are eligible to be reimbursed, in whole or in part, by any governmental program, such as patients enrolled in Medicare Part D and patients whose prescription is paid for by Medicare, Medicaid, Medigap, CHAMPUS, Department of Defense (DoD), TRICARE, Veterans Affairs (VA), Children’s Heath Insurance Program (CHIP), the Indian Health Service, or a state pharmaceutical assistance program. Blueprint Medicines reserves the right to rescind, revoke, or amend the program and discontinue support at any time without notice.
